New Patients

Welcome to the DMH Secure Patient Portal! 

Enroll as a New Patient -  Enrolling as a new patient is a two step process:

• Call our office at (217)876-5220 to confirm that we are accepting new patients and schedule an appointment.  You may then register as a patient on the website.

• Please arrive to your first appointment 30 minutes early to fill out paperwork. An alternative to expedite your first visit is to fill out the new patient demographics form and patient history online.  Please click on the links below to complete the demographics form and patient history form.
