1. Log onto  http://mydoctor.dmhhs.org
2. Select your MD  
3. Register.  Press the 'Register' Link in the upper left of the page. 
4. Terms of Use.  

          a)  I Accept 

          b)  I Do Not Accept 

    5.   Complete ALL fields on the page.   

    6.   Copy - Keep a copy of this information for your records.

    7.   Press SAVE Button   

    8.   Clinic Information - Identify yourself as a patient.  "Press I have been seen in the clinic as   a patient".    

    9.   Press NEXT at bottom of page.  

   10.  Identify Verification.
